RESUMO

INTRODUCTION: Video-based surgical coaching is gaining traction within the surgical community. It has an increasing adoption rate and growing recognition of its utility, especially an advanced continuous professional growth tool, for continued educational purposes. This method offers instructional flexibility in real-time remote settings and asynchronous feedback scenarios. In our first paper, we delineated fundamental principles for video-based coaching, emphasizing the customization of feedback to suit individual surgeon's needs. METHOD: In this second part of the series, we review into practical applications of video-based coaching, focusing on quality improvements in a team-based setting, such as the trauma bay. Additionally, we address the potential risks associated with surgical video recording, storage, and distribution, particularly regarding medicolegal aspects. We propose a comprehensive framework to facilitate the implementation of video coaching within individual healthcare institutions. RESULTS: Our paper examines the legal and ethical framework and explores the potential benefits and challenges, offering insights into the real-world implications of this educational approach. CONCLUSION: This paper contributes to the discourse on integrating video-based coaching into continuous professional development. It aims to facilitate informed decision-making in healthcare institutions, considering the adoption of this innovative educational quality tool.

RESUMO

Statistical analyses are an essential part of regulatory toxicological evaluations. While projects would be ideally monitored by both toxicologists and statisticians, this is often not possible in practice. Hence, toxicologists should be trained in some common statistical approaches but also need a tool for statistical evaluations. Due to transparency needed in regulatory processes and standard tests that can be evaluated with template approaches, the freely available open-source statistical software R may be suitable. R is a well-established software in the statistical community. The principal input method is via software code, which is both benefit and weakness of the tool. It is increasingly used by regulating authorities globally and can be easily extended by software packages, e.g., for new statistical functions and features. This manuscript outlines how R can be used in regulatory toxicology, allowing toxicologists to perform all regulatory required data evaluations in a single software solution. Practical applications are shown in case studies on simulated and experimental data. The examples cover a) Dunnett testing of treatment groups against a common control and in relation to a biological relevance threshold, assessing the test's assumptions and plotting the results; b) dose-response analysis and benchmark dose derivation for chronic kidney inflammation as a function of Pyridine; and c) graphical/exploratory data analysis of previously published developmental neurotoxicity data for Chlorpyrifos.

An investigation aimed at evaluating the ethical ed-ucation received by psychologists is presented. This research is part of the project "Ethical reasoning and judgement of psychologists in Iberoamerica, a study in Colombia". A multi-method study was applied, with focus groups in six professional areas, a sociodemo-graphic data questionaire and a scale evaluating ethics education, adapted from Pope et al. (1987). The scale was responded by 528 psychologists from different parts of the country, most of them graduated after the Act 1090 of 2006. Participants in the focus groups were 44 females and 17 males, with experience in the respective professional area. Results showed that 86.3 % of the participants qualified as good or excellent the ethics education in their graduate program, similar to the majority who received ethics education in their postgraduate or continued education programs. In the focus groups, commentaries about deficiencies in the academic programs were made, and also recom-mendations to orient ethics education towards situat-ed models, with previous preparation of professors. The importance of the role of Colpsic Colombian Psychologists College in the promotion of spaces for ethical analysis and discusión, and of the role of Ascofapsi Colombian Association of Psychology Programs in promoting an ethical education of quality in the psychology programs were emphasized. Results are discussed in the light of the wider project and the specialized literature

RESUMO

PURPOSE: The prevalence of depression in people with low vision is high and often goes undiagnosed. There is the potential for those who provide low vision services to perform concurrent depression screening. However, prior training in depression identification and suitable referral pathways is required. The aims of this study were: (1) to assess the impact of a training programme on practitioners' confidence and behaviour in addressing depression in patients with low vision, and (2) to review the training programme and identify areas for further development. METHODS: A convergent mixed methods approach was used. Questionnaires were completed by practitioners pre-, immediately post- and 6 months post- training (n = 40) to assess practitioner confidence in approaching depression in patients with low vision. Qualitative interviews were performed with a subset of practitioners 6 months post-training (n = 9). Additionally, routine data from the Low Vision Service Wales (LVSW) database was used to determine the change in the number of practitioners identifying depression in patients, and the change in the number of patients identified at risk of depression 6 months post-training. RESULTS: Of the 148 practitioners who completed low vision assessments pre- and post-training, 28 (18.9%) documented risk of depression in their patients pre-training, which increased substantially to 65 (43.9%) post-training (p < 0.0001). Mixed methods analysis confirmed increased documentation of depressive symptoms by practitioners. Practitioner confidence increased following training, with 92.3% feeling more confident to approach emotional issues with patients and 92.2% intending to use the recommended screening tool to identify depression. Interviews provided insight into areas where confidence was still lacking. Quantitative questionnaires revealed that training content was considered appropriate by 91% of participants. Interviews confirmed these findings while expanding upon possibilities for programme improvement. CONCLUSIONS: Training for depression screening was found to be time-efficient and acceptable for LVSW practitioners and shown to increase practitioner confidence in the identification of depression. Additionally, the programme changed behaviour, resulting in an increase in the identification of depression in patients with low vision. However, this is a complex topic and ongoing development is required to embed depression screening as an integral part of low vision services.

RESUMO

BACKGROUND: Many healthcare professionals, including pediatricians, psychiatrists, Clinical Psychologists (CPs), Occupational Therapists (OTs), and Speech-Language Pathologists (SLPs), are involved in the identification and intervention of Autism Spectrum Disorders (ASD) in children. Distinctive training backgrounds and professional exposure can result in contrasting ideas regarding the assessment, diagnosis, and treatment of ASD. Only a few studies have addressed the cross-disciplinary perspective of knowledge, belief, and awareness about diagnostic criteria required for diagnosing ASD. MATERIALS AND METHODS: A total of 154 allied healthcare professionals (98 SLPs, 33 CPs, and 23 OTs) participated in the study. The survey tool used for this study was adapted from a previously available survey on the assessment of knowledge and belief about ASD and self-efficacy. RESULTS: The overall knowledge and belief of allied healthcare professionals regarding ASD differed significantly across the groups. However, the knowledge of Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) diagnostic criteria for ASD did not differ significantly between the groups. CONCLUSION: Our findings have salient clinical implications and advocates for the continued education of healthcare professionals in India regarding recent diagnostic criteria for ASD.

CONTEXT: Cancer-related pain is associated with significant suffering and is one of the most challenging symptoms to manage. Studies indicate that front-line clinicians often lack the knowledge on best practices in cancer pain management. OBJECTIVES: The current project, a quality improvement (QI) initiative, evaluated the outcome of an online educational intervention for nurses on complex cancer pain management. METHODS: An online 7-module educational intervention, Advanced Pain Assessment and Management, was offered from 2012 to 2017. Pre-post course evaluations included self-reported knowledge and confidence across cancer pain management domains. In-course competency assessments included knowledge examination, online discussion forum participation, opioid dosage calculation assignment, and small-group-based case study. A mixed-model statistical analysis was used to assess pre-post course change in pain management confidence level. RESULTS: In all, 306 nurses from 89 hospitals in Ontario, Canada, were enrolled in the course; 81.4% returned the precourse survey and 71.9% successfully completed the course. The average confidence level on pain management was low at baseline (57.5%) but improved significantly post-course. In-course competency assessments ranged from 81% to 89%. Mixed-model results showed post-course improvements in confidence levels, independent of sociodemographic background, clinical role, and professional educational level. Nurses with longer years of practice and more cancer cases reported greater confidence. CONCLUSION: A facilitator-led online educational intervention focusing on complex cancer pain management can significantly improve nurses' knowledge, confidence, and skills. Low baseline knowledge among nurses highlights the pressing need for health-care organizations to implement cancer pain management training as an integral part of health-care QI initiative.

RESUMO

The European Union intends to enable its citizens to interact across borders in relevant areas of society and culture to further integrate neighboring regions. Medicine has not been at the core of recent EU-funded efforts in central Europe, partially due to significant differences in health care administration, delivery, reimbursement, and culture. However, impeding changes in social structure and centralization of specialized care warrant changes in preclinical administration of medical care, which are already transforming practices across developed countries in central Europe. Moreover, demographic and social changes are transforming not only patients but also health care providers, thus leading to an increased need for specialized medical personnel, particularly in regions close to formerly secluding borders. The EU-funded cooperation project presented in this article is located in the Euroregion Pomerania, which consists of northeastern Germany and northwestern Poland. This project emerged because of the need to solve practical emergency medicine-related problems for many years, which brought partners together. Unfortunately, administrative and medical interaction has not become significantly easier with Poland joining the Schengen area in 2007 and, subsequently, initial international contracts regarding, among other things, emergency medicine being negotiated and signed thereafter. Three different interdependent areas of cooperation within the project deal with key aspects of an improved and eventually integrated cooperation. An accepted clarification of administrative and legal foundations - or the lack and thus the need thereof - needs to be defined. Specialized language and simulation-based education and practice sessions employing modern technology throughout will be introduced to the entire region. Finally, the pre-existing and developing acceptance and sustainability aspects of personnel involved in the aforementioned actions and stakeholders on both sides of the border will be evaluated. In essence, the project focuses on a multimodal improvement of professional cooperation of key providers of emergency medicine services in the Euroregion Pomerania. Thereby, it aims to improve infrastructure; interpersonal and professional skills of involved personnel, administrative, and cultural relations; and eventually identification of specialized personnel with their workplace and region to secure and retain important medical workforce in an otherwise remote area on both sides of a formerly secluded border.

RESUMO

BACKGROUND: Currently, no data are available, which reflect the situation of medical doctors specializing in neurology in German hospitals. In order to secure the high standard of neurological patient care it is essential to evaluate the working conditions and the specialty training in neurology. OBJECTIVE: This nationwide survey was conducted throughout Germany with the aim to address problems and to give suggestions for improvements in neurological training curricula. MATERIAL AND METHODS: The survey was online from February to May 2017 and 953 neurologists undergoing further training participated. RESULTS: More than half of the young neurologists were satisfied with their medical training. One of the main problems that complicates clinical training is the workload. In addition, organizational obstacles within the clinic, such as poor structure of education or a lack of mentors, lead to dissatisfaction among participants. The size or type of the department, as well as the prevailing service system, exert only a minor influence on the quality of specialist training, although there were differences especially in the self-assessment of the participants in connection with the type of department (university hospital versus public or private hospital). CONCLUSION: Specialist training in neurology can be improved by simple arrangements, e. g., the introduction of a binding rotation scheme, internal mentoring and structured feedback. In addition, it will be necessary to relieve medical staff of administrative duties in order to create time for training and the learning of competencies.

ABSTRACT The study here reported had as objective to analyze the training experience in workers' and environmental health focused on local territory, adopting as interpretation main data assessments issued from the workers themselves. By means of content analysis, we arrived to two empirical categories of analysis. The first is interdisciplinarity and knowledge collective construction, and the second is intersectoriality and skills acquired through practice. At the end, formative elements of critical character were verified, which contribute to the emancipation of local players and actions of integrated public policies.

The present study has as its object the permanent education as a means to improve the quality of life in the healthy aging process of the workers from Escola Técnica Estadual do Estado do Rio de Janeiro (FAETEC) - the Marechal Hermes/RJ Campus. The general objective has been to use the permanent education with the workers from this state school, so as to contribute to a quality of life during the stages of healthy aging. The specific objectives were the following: identifying the needs of the school workers related to quality of life in the healthy aging process; describing the workers' perspective about the possibility of change in the healthy aging process; elaborating, together with the workers, creative and possible methods with respect to the social and economic reality in order to improve the quality of life in the healthy aging process. The methodology used was the Research-Action with a qualitative approach. The research's scenario was the language and computing center at the Escola Técnica Estadual do Estado do Rio de Janeiro (FAETEC), located in the neighborhood of Marechal Hermes, in the city of Rio de Janeiro. The research relied on four steps, and the data collection tools used were the following: 1st) Construction, by the researcher, from the servants' demand, of the pilot project "Labor Gymnastics" forwarded to management; 2) Semi-structured Questionnaire by which means the Bardin's content analysis was carried out; 3) Reconstruction of the "Labor Gymnastics" project with the workers' engagement, always aiming at the permanent education policy in health (PEPS) and Freire (2010) entitled "Labor Interaction Circle"; and 4) Presentation, approval and institutionalization of the new project in the managing board. Four categories must be pointed out as results which emerged from the questionnaires: 1) Identifying themselves with the permanent education in health, which either embraces the Permanent Education or recognizes its concept, given that, although this policy is not within the school environment, it is based on Freire (2010); 2) Recognition of the ignorance of knowledge by Socrates: "I know that I know nothing", which revealed that only 1% made a connection with either the continued education or did not understand or did not respond; 3) Learning to decide: healthy choices, which demonstrated that the participants recognized the meaning of the healthy aging process as quality of life; 4) Changes of paradigms: propose creative methods, which evidenced that many participants need an education policy which favors healthy habits with the objective to achieve a new life style. The products elaborated in this research were: a information material to move around through Whatsapp, pamphlets for distribution at schools and a banner to remain where the meeting of Labor Gymnastics: "Circle of Labor Interaction" took place. It has been concluded that the process of promoting the permanent education within the school environment demonstrated its innovative character, through the topic "quality of life, during all the steps of healthy aging". Therefore, it will enable a new demand to the SUS (Unified Health System), since the proposal is to move out from a biomedical model, in which there is a great medicalization, and aiming at the prevention of diseases such as hypertension, diabetes and obesities, start to promote a healthier lifestyle, so as to contribute to the changes of paradigms. We see the possibility of transforming the reality of the common sense into a critical sense, when dealing with physical, mental and nutritional health, with the improvement of the life quality of the workers involved in the research. This process will work as a powerful tool to promote health for the education workers themselves, and it is intended to expand to workers of other areas of the school

RESUMO

BACKGROUND: The physical inactivity pandemic and related non-communicable diseases have made it imperative for medical doctors (MDs) to effectively provide lifestyle counseling as part of prevention and treatment plans for patients. A one-day certification workshop was designed to improve MDs PA prescription knowledge, as part of the Exercise is Medicine® (EIM®) global health initiative. The objective was to determine knowledge gain of MDs participating in a standardized, one-day PA prescription workshop performed throughout Latin America (LA). METHODS: A 20-question multiple-choice test on PA topics, based on international guidelines, was completed before and after the workshop. Pre and post-test analyses, without a control group, were performed on 1044 MDs after the 8-h workshop that was delivered 41 times across 12 LA countries, from January 2014 to January 2015. Knowledge improvement was determined using the class-average normalized gain and individual relative gain. T-tests with 95% confidence interval levels were conducted to analyze differences between MD specialties. RESULTS: Test scores improved on average from 67 to 82% after the workshop (p <0.001). The average total individual relative gain was 29% [CI: 26 to 32%]. Relative gain by country ranged from 9.3% [CI: 2 to 16%; Nicaragua] to 73% [CI: 47 to 98%; Dominican Republic]. The mean of the 41 workshops' class-average normalized gain was 46% [CI: 42 to 51%]. The largest groups of participants were general practitioners (GPs) (33%; n = 348), internal medicine (19%; n = 194), and family medicine (9%n = 92) specialists. Relative gain for GPs was not different than for all grouped primary care specialties (30% vs. 27%, p =0.48). The knowledge gain was higher for the workshop modules on screening/risk stratification and prescription (43% [CI: 39-48%] and 38% [CI: 34-42%], than for the module on PA benefits and risks (26% [CI: 23-28%]). CONCLUSION: This one-day workshop had a positive impact on the knowledge gain of MD's on the topic of PA prescription. Although all groups of specialties increased knowledge, GPs and family medicine MDs benefited the most. This short course is an effective continuing education strategy for teaching PA assessment, counseling and prescription to MDs in Latin America, a topic rarely included in the training of MD's in the region and the world. Further follow-up is needed to ascertain impact on PA counseling practices.

RESUMO

Continuing education is the ideal mechanism to allow retired people to maintain their autonomy and health for as long as possible. Despite the importance of education in the development of human beings, very few studies have been identified on the need for continued education in older adults. This quantitative descriptive study allowed us to better understand the interests and the needs of seniors related to continuing education in French-speaking older adults in New Brunswick (n = 408), and to identify the perceptions of participants with regard to the benefits, the challenges and the preferences of seniors for continuing education. Study results indicate that the main interests of participants were for training in technology (computers), gardening and health matters. Given the lack of research in this domain, these results will be useful for future planning of continuing education initiatives for seniors.
